Materials that have already been published, distributed, or incorporated into completed records before withdrawal may remain in circulation or in WDSAI’s archives.

Applicant Confirmations

I.I confirm that I am currently associated with the institution named in this application in a faculty role.

II.I understand that participation in the Faculty Excellence Exchange does not itself confer Fellowship. Fellowship consideration follows completion of the applicable Exchange requirements and review of the participant’s record against the Faculty Practice Standards.

III.If selected to participate, I agree to attend the required sessions, submit the corresponding Implementation Notes within the prescribed periods, and complete the Practice Paper required for Fellowship consideration.

IV.I confirm that the information and materials I provide to WDSAI, including my name, institutional affiliation, academic information, and any photograph I upload, are accurate and that I am authorized to provide them. I understand that the name entered in my profile will be used for my WDSAI records and, if Fellowship is conferred, for my conferral document and entry in the Register of Fellows.
